WHAT IS YOUR PERSONAL REPORT CARD?
I HAVE BEEN USING THE PERSONAL REPORT CARD IN MY OFFICE FOR ABOUT FOUR YEARS.
I have found it to be a great tool to assess a patientโs habits and see how they progress over time. The 11 categories that I choose to grade are either the best habits or attributes that the healthiest/happiest people tend to have Aโs in.

On this page you will find a simple version of the Personal Report Card (PRC) that you can use to calculate your Health Point Average (HPA). Your HPA is just like a GPA for health.
How to give yourself a grade? If you havenโt read the book yet then here is a basic explanation.
Picture yourself as the head teacher for a student. The student, of course, is you. Letโs start with the first category, Diet. What grade would you give yourself if someone asked you how great your overall diet is? Would you give yourself an A? Are you more like a C+? The grade that you give yourself is very subjective. What that means is itโs based upon opinion. Your opinion of the quality of your overall diet. Still not sure what grade to give yourself? Your Health Point Average (HPA) is calculated by averaging your grade for each category. Your Health Point Average may range from 0.0 to a 4.0.
